Your APPLAUD team will ideally have 3-4 team members, though you may engage additional people to plan and implement the work. Depending on your context and goals, your team may be composed of physicians, medical office assistants, nurses and nurse practitioners, mental health/substance use service clinicians, and/or other allied health professionals. Please complete this section to the best of your ability; your team roster can be updated throughout the series and does not need to be confirmed at this point.
Leadership Support Contact
Receiving support from the person immediately in charge of the facility/service in which this team works is important to the success of this work. This person has the ability to allocate resources within the team and approve actions on-site to support the team’s improvement efforts and participation in the Action Series.

Participating in the Action Series

Compensation is available based on current sessional or hourly rates depending on the team member’s role. Funded activities will include live attendance to learning sessions (four 1.5-hour sessions) and simple data collection (1-hour per team each month). Funding will not be available for individual team huddles or time to engage in project work.

We want to evaluate the impact of the APPLAUD Action Series on AUD prescribing across the province. If your participation in APPLAUD is confirmed, do you agree to support your team to provide the relevant information for this evaluation?(Required)
To evaluate the impact of the action series, we will need some information from you/your prescribing team (i.e. prescriber ID and MSP billing number). This information will be used to create the outcome measures for the action series and ensure we’re only including the group of providers participating in the action series. The measures will only be reported as aggregated measures, not by individual providers. Prescriber IDs and MSP billing numbers will only be used for this purpose and won’t be shared.
