Position Summary

The Primary Care Networks (PCN) Coordinator plays a pivotal role in supporting the operational success of the PCN team by coordinating projects, streamlining processes, and ensuring alignment with the project’s strategic goals. This role requires a proactive, highly organized individual who thrives in a dynamic environment, with strong project coordination skills and proficiency in collaborative technologies.

Key Responsibilities

  • Assist in developing and implementing project work plans and coordinating PCN activities.
  • Track project timelines, deliverables, and action item progress, gathering updates from stakeholders and reporting internally.
  • Proactively identify potential project risks or inefficiencies and proposes solutions.
  • Streamline and optimize operational processes and procedures.
  • Maintain and organize shared project documentation including ensuring databases, documents, templates, and records, are up-to-date and accessible.
  • Develop and distribute communication materials; responding to inquiries from health care providers, PCN team members, stakeholders, and the public.
  • Support change management initiatives related to PCN implementation.
  • Independently manage meeting schedules, prepare agendas, and facilitate follow-up actions.
  • Assist with onboarding new team members, including training on processes, tools, and technologies.
  • Create engaging virtual work environments.
Qualifications:
  • Post-secondary degree or diploma program.
  • Minimum of five years of recent, related experience.
  • Experience within the health care sector is considered an asset.
  • A combination of relevant education and experience will be considered.
  • Valid driver’s license and access to a reliable vehicle preferred.
Skills and Abilities:
  • Proven ability to coordinate and track project timelines, deliverables, and action items.
  • Strong interpersonal skills with professionalism, diplomacy and integrity.
  • Excellent organizational and time management skills with attention to detail
  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook), collaborative tools such as Zoom, Microsoft Teams, SharePoint, and OneDrive, and survey tools.
  • Familiarity with project management tools (e.g., Wrike, Smartsheet, Asana) an asset.
  • Strong written and verbal communication skills tailored to diverse audiences.
  • Ability to prioritize multiple tasks in a fast-paced, dynamic environment while meeting competing deadlines.
  • Self-starter who can troubleshoot, recommend solutions, and collaborate effectively.
  • Experience supporting Indigenous cultural safety, reconciliation, and diversity, equity, and inclusion (DEI) initiatives.
  • Strong analytical skills for identifying and improving efficiencies.
  • Passion for supporting the health care field.
Working Conditions:
  • This is a full-time, term position (37.5 hours per week) through March 31st, 2027.
  • This is a remote/ hybrid work position with the occasional in person meeting requirement for team meetings and on-site work at various locations across the region.
  • Successful candidate must have access to a home office or other remote work location.
  • The successful applicant must reside in, or close to, the Thompson Region to facilitate travel within the region where necessary. Additional travel may be required.
  • Equipment such as laptop will be supplied to the employee.
